Open Singles	$15.00	________	All entrants must be members of the USATT or have Player Permit. If not a current member you must either buy a membership (adult $40 1-yr or $105 3-yr, Junior/Student 1-yr $20) or get a one-time Player Permit ($10). All entrants must also pay the Rating Fee for which they will receive a national rating. Fees will be refunded if event cancelled because of insufficient entries, or if maximum number of players already entered.

Entries must be received by 6:00 p.m., Sat, April 11, 2009. Late entries will only be allowed if space permits, and a $1 late fee per event will be assessed. If you got a one-time permit in the past, you must be a full member for this tournament.

I will sign at the tournament the USATT comprehensive waiver of liability and I also release the sponsors from all responsibility for any accidents that might occur while participating in this event.
